RESUMO

This paper describes the beginning of the implementation process of the health impact assessment in the Valencian Community (Spain), as an instrument to incorporate the framework of social determinants and health inequalities in the policies issued by the different departments of the Valencian government. The proposal involves: 1) political commitment, with legislative and strategic planning actions; 2) the creation of structures to allow intersectoral collaboration, with the establishment of the health impact assessment commission and the intersectoral technical committee; and 3) the design and validation of a tool for the simplified health impact assessment of non-health policies adapted to the Valencian Community. We highlight the importance of the participatory methodology used in the whole process and the potential of the health impact assessment for the development of public policies oriented to improve health and equity.

RESUMEN Objetivo Identificar los principales objetivos y énfasis de los proyectos de investigación en malaria financiada por Colciencias entre 1995 y 2005. Métodos Para la identificación del énfasis de los proyectos de investigación en malaria se utilizó el sistema de clasificación en investigación en salud desarrollado por la Colaboración de Investigación Clínica de Gran Bretaña: "Health Research Classification System" (HRCS). Resultados Se encontró que la investigación en malaria durante este periodo tiene énfasis en determinantes y causas, las principales ciudades beneficiadas con la financiación de proyectos de investigación en malaria son instituciones pertenecientes a Bogotá, Medellín y Cali. Conclusiones Estos proyectos permitieron el fortalecimiento en núcleos de investigación en malaria en las líneas de biología molecular, producción de vacunas, identificación de resistencia a antimaláricos y estudios relacionados con el cambio climático; algunos de sus resultados con capacidad de afectar políticas en malaria y formación de recurso humano.(AU)


ABSTRACT Objective To identify the main focus of research projects on malaria funded by Colciencias between 1995 and 2005. Methodology The Health Research Classification System (HRCS), developed by The UK Clinical Research Collaboration (UKCRC), was used for identifying the emphasis of research works on Malaria. Results During this period, research on malaria was mainly directed to etiology; the main cities benefited by the funding of research projects on malaria are institutions from Bogotá, Medellín and Cali. Conclusions These type of projects improve the strengthening of malaria research centers in areas such as molecular biology, production of vaccines, identification of resistance to antimalarial treatment, and climate change; some of the results obtained can affect policies and training of human resources on malaria.(AU)

RESUMO

La investigación en salud incide sobre la salud de las poblaciones. El presente ensayo realiza un acercamiento a los marcos y modelos empleados por los países desarrollados para la evaluación del impacto de las investigaciones en salud, por medio del análisis documental. De esta manera se identifican dos ejes orientadores de análisis: uno de enfoque y el otro de énfasis, en los que se relacionan los modelos publicados, sus orígenes, sus alcances y sus usos. Esto permite conocer las características que poseen y los ámbitos en los cuales Colombia podría implementarlos. El modelo de evaluación "retorno de la inversión" es un modelo que permite seguir el proceso y los resultados de investigación, por medio de la categorización multidimensional de los impactos de la investigación.(AU)


Health research produces effects on the health of populations. This document approaches the frameworks and the models used by developed countries to assess the impact of health research through documentary analysis of research with the highest impact. With this, it was possible to identify two guiding axes of analysis: one having to do with focus, and the other having to do with emphasis. With these, the published models, their uses, their reach, and their origins are related. Our study brings awareness to the features they have and the areas in which Colombia could implement them. We found that the framework for evaluating health research known as the "payback model" is a model for monitoring research that tracks the process and research results with multidimensional categorization of the impacts of research.(AU)

AIM: To determine the health impact perceived by residents and social players involved in two urban regeneration interventions (a new fish market and the redevelopment of North/West Herrera) in Pasaia Bay (Gipuzkoa, Spain) that have been the subject of a health impact assessment (HIA). METHOD: Qualitative methodology was used with theoretical and intentional sampling. Information was obtained through 18 personal interviews and five discussion groups and was analyzed in accordance with the sociological analysis model of discourse. The preliminary results were triangulated and contrasted among the team members and those taking part in the study. RESULTS: Four interrelated areas of health impact were identified: urban quality, connectivity, social cohesion, and-to a lesser extent-employment. Specific aspects for improvement were indicated for each field, as well as the influence of the sociopolitical context and conceptions of health. Other significant findings were the impact of the process of carrying out the building work and the distinct perspectives due to the differing roles and social profiles of participants. CONCLUSIONS: Knowledge of the perceptions and expectations of affected individuals through qualitative methods provides novel elements and interrelations that are needed to apply HIA as a tool for improving health and for citizen participation.

RESUMO

Health states are the result of the effect of multiple social determinants of health (SDOH). Health inequities appear as a consequence of the adverse interaction of the SDOH, leading to avoidable and therefore unfair health disparities between and within populations. Although the European population achieves higher levels of health and life expectancy than ever before, health inequities between and within European countries are still widespread and even increasing in some areas. The current economic crisis further shows significant negative impacts on the SDOH and consequently on the health of populations. Furthermore, data suggests that governmental responses of several European countries to the crisis failed to provide sustainable and comprehensive solutions as they do not take health into consideration. However, strong economic, social and health systems seem to act preventatively on negative effects on SDOH and health itself. Moreover, intersectoral governance structures and Health Impact Assessments (HIA) can foster the narrowing of unfair health gaps.


Los estados de salud son resultado del efecto de los múltiples factores sociales determinantes de la salud. Las inequidades en salud aparecen como consecuencia de la interacción adversa de dichos factores determinantes, que llevan a disparidades en salud entre diversas poblaciones y entre integrantes de una misma población, las cuales pueden catalogarse como injustas y evitables. Aunque la población europea ha alcanzado altos niveles de salud y esperanza de vida como nunca antes, las inequidades en salud entre y dentro de los países europeos se encuentran aun ampliamente extendidas y en incremento en algunas áreas. La crisis económica actual, además, muestra impactos negativos significativos sobre los factores determinantes sociales de la salud y, por consiguiente, sobre la salud de las poblaciones. Adicionalmente, la información actual sugiere que las respuestas gubernamentales de varios países europeos ante la crisis fallaron en su objetivo de proveer soluciones integrales y sostenibles, en la medida en que estas no toman en cuenta a la salud. Sin embargo, los sistemas de salud, sociales y económicos sólidos parecen actuar preventivamente ante los efectos negativos sobre los factores sociales determinantes de la salud y sobre la salud misma. Por otra parte, el desarrollo de estructuras gubernamentales intersectoriales y de estrategias como la evaluación de impacto en salud, pueden fomentar la reducción de disparidades en salud consideradas como injustas.
